Private Function Equipment Reservation Form

Fill out the form below to reserve your equipment.  Payment must be received to complete the registration.  After submitting this form you will be taken to a PayPal payment page where you can pay with PayPal, Visa, Mastercard, American Express or Discover cards.

***This form must be completed by individual, parent or guardian 18 years old or older.

This form is for private event equipment rentals only not normal equipment rentals during normal hours.

Upon arrival at Lost Valley, please head down the stairs on your right before entering the lodge, to get to our rental area.  Give your name to the attendant and they will issue your equipment.

Any guests under 18 must have an individual 18+ sign the waiver on their behalf when picking up the equipment
